Unit aim

Provides students with the principles of effective care planning and case management for Indigenous peoples in urban, rural and remote settings. The importance of linkage to accessible, culturally competent services and supporting individuals and families with effective self-care strategies for chronic and acute health conditions is explored.

Unit content

Topic 1: Appropriate and effective case management for Indigenous clients
Topic 2: Culturally competent care planning
Topic 3: Improving linkage and collaboration between mainstream health services and Aboriginal controlled health services
Topic 4:  Strategies for effective communication in planning and care between hospital staff; multi-disciplinary teams; primary care clinicians; Aboriginal specific services; families and clients
Topic 5: Developing, implementing and evaluating effective care plans within complex care services across multiple organisations

Learning outcomes

On completion of this unit, students should be able to:
1 apply learning from this unit to strategies for service provision to Indigenous clients with chronic and acute conditions
2 compare and evaluate effective case management and care planning strategies which support individuals and families with complex or challenging health conditions
3 appraise the cultural competence of case management and care planning programs for Indigenous clients and their families.

On completion of this unit, students should be able to:
